Your Appointment Checklist: Read Before You Leave!
Please go through this list to ensure a smooth experience at the Embassy.
1. The "Must-Haves"
[ ] Physical Folder: All original documents plus your 3 sets of black & white copies.
[ ] Passport Photo: 5x5 cm (2x2 inches) on a white background.
[ ] Previous Passports: Any current or expired U.S., Israeli, or foreign passports.
[ ] Payment Confirmation: The printed receipt/screenshot from Pay.gov.
[ ] Appointment Confirmation: The printed page showing your time and date.
2. Critical Reminders
[ ] Do NOT Sign: If you are filling out a DS-11, leave the signature line blank! You must sign it in front of the Consular Officer.
[ ] Black Ink Only: Ensure any forms you filled out by hand use black ink.
[ ] Arrival Time: Arrive 15–20 minutes early for security screening.
[ ] Who is Coming? * For Minors: The child and both parents must attend.
If one parent cannot attend: You must bring the original, notarized DS-3053 form and a copy of that parent’s ID.
3. Security Note
The Embassy does not allow large bags, laptops, or liquids.
Cell phones are usually collected at security and returned when you leave.
