Your Appointment Checklist: Read Before You Leave!


Please go through this list to ensure a smooth experience at the Embassy.

1. The "Must-Haves"

  • [ ] Physical Folder:Ā All original documents plus your 3 sets of black & white copies.

  • [ ] Passport Photo:Ā 5x5 cm (2x2 inches) on a white background.

  • [ ] Previous Passports:Ā Any current or expired U.S., Israeli, or foreign passports.

  • [ ] Payment Confirmation:Ā The printed receipt/screenshot from Pay.gov.

  • [ ] Appointment Confirmation:Ā The printed page showing your time and date.

2. Critical Reminders

  • [ ] Do NOT Sign:Ā If you are filling out a DS-11, leave the signature line blank! You must sign it in front of the Consular Officer.

  • [ ] Black Ink Only:Ā Ensure any forms you filled out by hand use black ink.

  • [ ] Arrival Time:Ā Arrive 15–20 minutes early for security screening.

  • [ ] Who is Coming?Ā * For Minors:Ā The child andĀ both parents must attend.

    • If one parent cannot attend:Ā You must bring the original, notarized DS-3053Ā form and a copy of that parent’s ID.

3. Security Note

  • The Embassy does not allow large bags, laptops, or liquids.

  • Cell phones are usually collected at security and returned when you leave.
