What forms do YOU need for your renewal appointment?

Minor Renewals:


  • DS11 - Filled in Online or by hand https://eforms.state.gov/Forms/ds11.pdf

  • Passport photos - 3X - taken within the last 6 months

  • Both parents need to be there, if not, fill this out with a notary - Form DS3053 https://eforms.state.gov/Forms/ds3053.pdf

  • The minor’s original birth certificate or original Consular Report of Birth Abroad is required to prove the parental relationship.

  • CC payment

2. Photo: One 2 inch x 2 inch / 5cm x 5cm square passport photo taken within the last six months.

Note: There is no photographer at the Embassy. There are multiple locations to get a passport photo near the Branch Office in Tel Aviv.

3. Evidence of U.S. citizenship: Original documents are required. Notarized copies are unacceptable. Bring one photocopy.

4. Proof of Identity: Original documents are required. Notarized copies are unacceptable. Bring one photocopy. Must be a valid, government-issued photo I.D., such as:

  • Foreign passport

  • Driver’s License

  • National identity card with photograph, or

  • Military I.D with photograph

5. You can pay in advance here: https://www.pay.gov/public/form/start/790155895 and bring proof of payment, OR you can pay in person via international credit cards - or Israeli credit cards. No bank cards, no debit cards, no checks.
