How to prep for a first adult passport.

Prepare your forms and documents:
▪︎ DS-11 / DS-64
▪︎ Passport photos (3)
▪︎ Old Passport and 3 photocopies of it
▪︎ Foreign passport and 3 photocopies of it
▪︎ ID card and 3 photocopies of it
▪︎ ID card of both Mother & Father and 3 photocopies of each
▪︎ US Birth certificate and 3 photocopies
▪︎ Payment by Cash or Credit Card 195$
2. Photo: One 2 inch x 2 inch / 5cm x 5cm square passport photo taken within the last six months.
3. Evidence of U.S. citizenship: Original documents are required. Notarized copies are unacceptable. Bring one photocopy.
U.S. birth certificate
A valid or expired U.S. passport (with your signature)
4. Proof of Identity: Original documents are required. Notarized copies are unacceptable. Bring one photocopy. Must be a valid, government-issued photo I.D., such as:
Foreign passport
National identity card with photograph, or
How to Show Parental Awareness:
A legal parent or legal guardian appearing with you in person when you apply for your passport. The agent or employee accepting the application will ask your legal parent or legal guardian to sign Form DS-11. Bring a photocopy of that parent or guardian's ID.
Submit a signed note from your legal parent or legal guardian with a photocopy of their ID.
We may ask you to submit a notarized statement from your legal parent or legal guardian (e.g., on Form DS-3053) which states he or she supports you receiving a passport. The statement must be accompanied by a photocopy of that parent or guardian's ID.
