Adults
1. Go fill out your form here: DS-82

2. Print it.
3. Put the paperwork, your old passport, and the proof of payment sheet in a Manila envelope.
4. Get a new passport photo taken.
5. Make a Photocopy of an additional photo ID.
6. Put a Self addressed envelope inside. Add your phone number to that address.
7. Submit it.
Minors
